Freedom of Information Act (FOIA) Requests

The Lake Villa Fire Protection District requires all requests to inspect or copy public records to be submitted to us in writing.  We encourage requests to be submitted on the form provided via the link located on this page, or in the District Office.  The Lake Villa Fire Protection District FOIA Officer will review all written requests in any form.

The FOIA Officer will respond to each written request to inspect, copy or both in a manner consistent with the Illinois Freedom of Information Act.  Requests made for all commercial purposes will be responded to within 21 business days upon receipt.  All other requests will be responded to within 5 business days from the day after the request is received.​

  • Exact location

  • Name and D.O.B. of patient

  • Date of Incident

  • How you would like the information sent (via email, fax or mail)
