I just received an ambulance bill; I am a resident of Lake Villa/Lindenhurst, do I have to pay?
You must first submit your bill to your health insurance company. If there is a balance and you are a resident of either Lake Villa or Lindenhurst, you may submit your bill and E.O.B. from your insurance to be reviewed. You will be contacted within 48-72 hours confirming approval or denial of your request.
Submit requests for review, including your bill and E.O.B. to: lvfpd@lakevillafpd.com
Non-residents will need to contact either EMS/MC Medical Billing or PBS (see immediately below) directly to arrange payment of the ambulance bill.

Date of Service Prior to March 2025: Contact EMS/MC Medical Billing at (866) 827-8469 for any billing questions.
Date of Service After/During March 2025: Contact Paramedic Billing Services (PBS) for any billing questions at (888) 461-2595 or at the PBS Website.

We work directly with IOS (Industrial/Organizational Solutions) which is a human resources consulting firm and test publisher, specializing in personnel selection for public safety agencies. Tests are performed periodically; after testing, a list will be made available to the hiring officers at LVFPD. Candidates will be contacted based on testing results.
